Complete this section if applying to be a Counsellor or Counsellor in Training. Campers have varying abilities and skills. It is helpful to us if you would give us a sense what type of Camper you would like to develop a friendship with; for example you might be matched with a highly active, non-verbal, Camper, or a non-active, verbal Camper, or a highly social, non-mobile Camper. What kind of friend would you like to have for the week? We will attempt to peer match you with a Camper that has the characteristics you choose.

Volunteer Participation Agreement

If the volunteer is younger than 18 years of age, this form must be completed by a parent or legal guardian.

The volunteer wishes to participate in the Rehoboth Camp Ministry. As a condition thereof, he/she agrees to the following:

  1. A variety of activities will be offered in the camp program, and some of them may present stresses and hazards that are unforeseeable. The volunteer assumes all ordinary risks involved in participating in these activities, and agrees that Rehoboth Christian Ministries, its representatives, and any persons conducting the activities will not be responsible for any damages or injuries to the applicant in the absence of evidence of gross negligence.
  2. Rehoboth Camp Ministry staff reserve the right to dismiss the volunteer from further participation in the camp program if he/she is deemed unsuitable for this type of camping experience in accordance with the Code of Conduct. The volunteer or his/her parent/legal guardian will be responsible for any additional transportation costs resulting from such a dismissal.
  3. The volunteer releases Rehoboth Christian Ministries and its representatives from all liability for personal injury resulting from:
    • any participants failure to obey, to the best of their ability, the safety regulations and directions of activity leaders; or
    • the exercise of judgment by activity leaders made in good faith in response to emergencies that may occur.
  4. Rehoboth Christian Ministries and its representative will not be liable for loss of, or damage to, the volunteer’s personal property in the absence of willful misconduct by them.
  5. Any medical costs incurred by the volunteer will be his/her sole responsibility, or his/her parent/legal guardian.
  6. The volunteer or his/her parent/legal guardian will be liable for damages to property or facilities of Rehoboth Christian Ministries that result from the volunteer’s actions.
  7. Rehoboth Christian Ministries has the volunteer’s permission, or that of his/her parent/legal guardian, to
    • photograph or create audio-visuals of the volunteer’s image, work, or performance; and
    • to have these works displayed, published, or distributed for advertising, promotion, or publicity purposes by Rehoboth Christian Ministries without compensation to the volunteer.
  8. A Registered Nurse (RN) provides medical support for the camp program. The volunteer agrees this is sufficient coverage to meet his/her medical needs.
  9. The RN will authorize any emergency medical and/or hospital care for the volunteer. The Emergency Contact person named in the volunteer’s camp application will be notified as soon as possible in the event of any emergency affecting the volunteer.
  10. Rehoboth Christian Ministries may reject any volunteer applicant should he/she or the parent/legal guardian fail or refuse to complete and sign all required application documents.
