- COMMUNITY PARTICIPATION SUPPORTS (VOCATIONAL PROGRAM)
Rehoboth's Community Participation Supports is designed to provide flexible and realistic working environments in which clients may discover and develop their individual interests and aptitudes. These environments also provide opportunity for clients to learn appropriate work habits and to pursue their individual vocational ambitions and dreams.
- The Community Participation Supports Program provides three main components. Each client is given the opportunity to develop an Individual Life Direction Plan which encompasses their dreams and goals for their lives. Dependent on this Plan the following components may be combined or modified to enable the client to move toward attaining their personal goals. The three main components are as follows:
1. Employment Preparation
The Employment Preparation program offers an individualized day placement for clients requiring training to accomplish, maintain and generalize work skills. The program will focus on attaining work skills. This program will include: a variety of facility based or community based work and ongoing job readiness training and instruction. The expected outcome of this program is participation in the work force either with or without support staff. A volunteer activity may also be considered an appropriate component of employment training if participation is time-limited.
2. Employment Placement
The Employment Placement program offers an individualized community based program for clients that are able to pursue competitive employment opportunities. Referrals to this program may be external or internal to the agency, depending on individual information and skills. This program will : job search and procurement, intensive support and training in the community setting, and supports for as long as required to maintain such a placement. The expected outcomes for this program would include: support ad assistance in maintain employment or self-employment, the individual would receive remuneration for the job performed, and has components such as on-the-job support, consultation with supports, supports assistance with employment stabilization and employment maintenance.
3. Community Access Supports
The Community Access program offers an individualized day placement providing supports to clients who desire participation in work, activities and personal development. This program will include: instruction in basic work skills, community involvement, integration, socialization, recreation, leisure, street and safety training, all of which will utilize a variety of work site and community settings. Community Access is a separate program from the residential program determined by the goals and desires of the individual. It provides a valued alternative to employment and facilitates community inclusion. There may be a supported or independent volunteer outcome as a valued contribution to the community.
- Service Delivery
A Rehoboth Planning Facilitator will be assigned to each client accepted into services. It is the responsibility of the Planning Facilitator, in conjunction with the client, their guardian and their support network, to develop an Individual Life Direction Plan. This Plan promotes the philosophy of person centered planning and thus provides services that are in accordance with the desires of the individual. In addition to the person centered plan, the Planning Facilitator may also consult with the client and guardian on the use of assessment tools to identify key skills, competencies and skill areas. Assessment may assist in the following areas: identifying client interests, understanding the skills they may need to function effectively in the community, and determine the level of support they may require in the activities they choose. Information from the assessment will assist in the development of the Individual Life Direction Plan.
- Each client will also be assigned a Key Worker. The Key Worker is ultimately responsible for ensuring that services are being provided to the client as per the Individual Life Direction Plan, for communicating with the Guardian, and for ensuring the health and well-being of the client. Employees working with the client will be responsible for maintaining accurate documentation on their progress.
- The client's Key Worker (Rehoboth employee) is responsible for the completion of a monthly summary as well as ensuring that they have a monthly meeting with the client. The Planning Facilitator will attend this meeting as needed. The Key Worker will review progress, needs and desires of the client and will address concerns and accomplishments as they arise.
- Incentive
If the client's program includes a facility based production work site, they are entitled to receive an incentive based on the individual skill level and production output. The incentive range is from $0.25/hour through $1.25/hour. The incentive is provided to defray costs which may be incurred during training and does not constitute remuneration.
- Transportation and Program Expenses
The availability and need for public transportation varies from region to region. Rehoboth will make every attempt to facilitate community inclusion, including transportation where necessary. If transportation is provided by Rehoboth, the client will be charged a monthly fee which will be outlined in the Service Agreement.
- Clients accessing the Community Access component of the program may be required to cover expenses incurred in scheduled recreation and leisure outings. This will be discussed prior to entrance into the program.
- Hours of Operation
The Employment Preparation and Community Access components of the program are offered Monday through Friday, closing for all statutory holidays.
- The Employment Placement component operates according to the requirements of the employer with whom the placement has been secured,
- Note: There is some flexibility in the hours for all components of the program based on the individual needs of the client.
- It is requested that clients provide their supervisor with one-month advance notice for, planned vacations, upcoming scheduled appointments, etc.
