STATEMENT OF PURPOSE
- Rehoboth's Residential Program will endeavor to create meaningful home environments that recognize the potential of individuals and provide them opportunities for continued growth, personal and spiritual development and life long learning.
- Individuals interested in Residential Services will be required to follow Rehoboth's standardized Admission process.
- The process of applying for support funds through Persons with Developmental Disabilities Community Boards (PDD) varies from region to region. Rehoboth will assist the individual and/or guardian in attaining the necessary funding. . When an application for Residential Services has been accepted, Rehoboth will immediately begin the process of "peer or roommate matching".
- The current residents or potential roommates and their families will be introduced over a series of meetings and social settings to determine compatibility. All individuals involved must agree to the match prior to service commencement.
COMPONENTS OF RESIDENTIAL SERVICES
- Rehoboth provides 24-hour residential supports in both urban and rural communities. The services are provided in both Rehoboth owned properties as well as leased and rented properties. Rehoboth promotes and encourages choice in the location of the residence, the furnishing and the routines of the home. Each resident will be expected to cover an equal portion of the living costs.

- While the Service Delivery system promotes the philosophy of person centered planning and the use of an Individual Life Direction Plan, the Planning Facilitator may, in consultation with the client and Guardian, recommend an assessment tool to identify key skills, competencies and goal areas. Assessment may be used for the following reasons:
- To assist the client in identifying interests
- To assist the client in understanding the skills they may be required to demonstrate in the community
- To determine the level of assistance required to support the client
- To provide current information about the client in areas that will contribute to the development of the Individual Life Direction Plan
- Each client is assigned a Key Worker. The Key Worker is ultimately responsible for ensuring that services are being provided to the client as per the Individual Life Direction Plan, for communicating with the Guardian, and for ensuring the health and well being of the client.
- The level of support (staffing model) is dependent upon the needs of the residents. Rehoboth will work with the individuals and their families to determine an appropriate level of support and then negotiate the funding with Persons with Developmental Disabilities Community Boards (PDD). Rehoboth is responsible for the hiring, orientation and ongoing evaluation of all employees.
SERVICE DELIVERY
- A Rehoboth Planning Facilitator will assigned to each client accepted into services. It is the responsibility of the Planning Facilitator, in conjunction with the client, their guardian and their support network, to develop an Individual Life Direction Plan to assist in ensuring that the services Rehoboth provides are in accordance with the desires of the individual.
