Residential Direct Service Worker(DSW)/CRWI

MAIN PURPOSE:

This is a front line position, which provides training, care and support to the clients of residential programs in accordance with the organizations philosophy, policies and procedures.

KEY RESPONSIBILITIES:

  • Participates in the development and implementation of Individual Life Direction Plans for clients
  • Provides support and advocacy for clients
  • Provides personal and health care services, while promoting client independence
  • Encourages client socialization and ensures enrollment in desired activities
  • Provides training and supervision in basic life skills and community/family living skills
  • Ensures that community services are made available to clients and assists clients in accessing community services
  • Ensures a safe, clean and comfortable living environment by participating in regular household routines and assisting clients in household routines
  • Maintains and updates required documentation and reports in accordance with the organizations policies and procedures
  • Establishes and maintains open, timely and appropriate communication and relationships with clients, families/guardians, community and other staff
  • Maintains professional currency in the community rehabilitation field
  • Ensures that all client medications are administered according to Rehoboth's Medication System and Administration
  • Attends and participates in all required meetings as requested by Team Leader
  • May have Key Worker responsibilities which includes: primary support person to client, primary contact with guardians, parents and community members, assesses and assists with general health needs, personal needs, and daily living needs of clients, provides ongoing feedback and documentation regarding skill development and required supports, coordinates recreational, special events and transportation, monitors clients’ funds and completes financial statements.   • Performs other duties as assigned by Team Leader

AUTHORITY AND SCOPE OF OPERATIONS:

The Direct Service Worker reports to the Team Leader of Residential Services. The DSW is responsible for the every day occurrences within the residential setting and implements to Individual Life Direction Plans of the clients. This person works within a team setting.

SKILLS REQUIRED:

Education: Rehabilitation Practitioner Diploma or related human services field, or

Basic Skills Training

Emergency First Aid and CPR

Class Four License or One million dollar liability insurance (when applicable)

Non Violent Crisis Intervention Training (when required)

Medication Administration

Client Rights

Abuse Prevention & Response Protocol

Years of Experience: Some experience in a related field, experience working with persons with disabilities

Interpersonal: Professionalism, maturity, ability to relate well to staff and clients, demonstrated communication skills

Hours of Work: 100% Direct Service, mainly evenings, nights and/or weekends

  • Client satisfaction
  • Family/support team satisfaction
  • Consistent implementation of Individual Life Direction Plans
  • Ability to follow directions
